แบบฟอร์มที่กำหนดแล้วส่งตรงถึงผู้อำนวยการได้ทันที หนังสือร้องเรียนทุกฉบับจะได้รับการพิจารณา และรายงานผลให้ท่านทราบทาง E-mail หรือทางหมายเลขโทรศัพท์ ที่ท่านกรอก
